Personalized Secretome Therapy vs. Lab-Derived Exosome Therapy: The New Frontier of Hair Restoration
By Dr. Kim Meathrel, FRCSC — Plastic Surgeon, Bespoke Hair MD, Kingston ON
Redefining Hair Regeneration
Hair restoration is rapidly evolving from traditional treatments like PRP and medications to the next generation of regenerative therapies that use the body’s own biology to repair and regrow hair.
Two of the most exciting developments are secretome therapy and exosome therapy — both based on cellular communication, but very different in how they are created and delivered.
At Bespoke Hair MD, we offer personalized autologous secretome therapy, designed from your own follicles, for a completely individualized regenerative treatment.
What Exactly Is a Secretome?
The secretome refers to all of the regenerative molecules that a stem cell releases — a blend of growth factors, cytokines, and tiny extracellular vesicles that stimulate repair and new growth.
In hair restoration, we can actually harvest a patient’s own tissue (a few healthy hair follicles) and grow out the cells in a controlled environment at our partner lab in Toronto. Over time, these cells release their unique collection of regenerative molecules — your personal secretome.
Once collected and concentrated, that secretome can be re-introduced into your scalp through precise microneedling. Because it originates from your own biology, it carries your personal molecular signature and communicates perfectly with your own hair follicles.
Why It Matters
100% autologous — no donor or synthetic material
Personalized to your genetic and cellular profile
Designed to restore the same follicles it came from
This is the ultimate example of bespoke medicine: your own cells, re-trained to restore your hair.
What Are Exosomes — and How Are They Different?
Exosomes are microscopic, bubble-like particles (30–150 nm) that naturally occur within every secretome. They are the delivery vehicles that carry genetic instructions (like RNA and proteins) between cells to trigger repair.
However, the commercial exosome products used in many hair clinics are not made from your own cells. Instead, they are lab-manufactured, typically derived from:
Plant stem cells (such as wheat, green tea, or ginseng cultures)
Or allogeneic human sources (donor stem-cell lines expanded in a lab, not allowed in Canada due to biologic risks)
These exosomes are purified, standardized, and mass-produced — meaning they are generic biologic signals, not specific to your body. Secretomes contain your personal exosomes, Exosomes do not contain secretomes.
Key Difference: Personalized vs. Generic
Personal Secretome Therapy | Lab-Grown Exosome Therapy | |
Source | Your own hair follicles or tissue | Plant or donor stem-cell cultures |
Type | Autologous, biologically matched | Allogeneic or botanical, generic |
Regenerative Content | Full range of growth factors, cytokines, vesicles and exosomes | Only purified exosomes |
Immune Compatibility | Fully compatible (from you) | May be neutral or less specific |
Purpose | Personalized tissue regeneration | General scalp rejuvenation |
Regulatory Pathway | Autologous biologic, performed under physician supervision | Lab-manufactured |
At Bespoke Hair MD, we believe in a science-first approach: when possible, using your own biology for a treatment that is truly tailored to you.
How Personalized Secretome Therapy Works
Collection: A few hair follicles are obtained in our Level 3 CPSO-accredited facility.
Cultivation: The cells are nurtured in a sterile culture system at our partner lab in Toronto General Hospital, encouraging them to secrete a regenerative “cocktail” of growth factors, cytokines, and vesicles — your secretome.
Harvesting: The secretome fluid is collected and processed into a cell-free concentrate. 12 vials of your secretome is then shipped back to the clinic for your treatment.
Treatment Delivery: Using microneedling, your secretome is applied to areas of thinning to reactivate dormant follicles and improve scalp health.
Follow-Up: Most patients receive 3-6 sessions spaced one month apart, with maintenance sessions.
How Exosome Therapy Fits In
Exosome therapy can be a useful adjunct or regenerative option for those not ready for cell collection. Plant-derived or donor-based exosomes may:
Boost scalp hydration and blood flow
Provide short-term follicle stimulation
Be combined with PRP or microneedling for added benefit
However, because these vesicles are not individualized, their signaling may be less targeted and their duration of benefit shorter compared to an autologous secretome.
Note: human donor based exosomes are not allowed in Canada.
Secretome and Exosome Therapies vs. PRP
Treatment | How It Works | Ideal For |
PRP / PRF | Growth factors from your own platelets; short-term repair | Early thinning, younger clients |
Secretome Therapy | Full range of regenerative factors from your own follicle cells | Personalized regenerative therapy |
Exosome Therapy | Pre-made vesicles from plant or donor cultures | General scalp rejuvenation or maintenance |
FUE Hair Transplant | Surgical redistribution of follicles | Advanced hair loss |
Each method has a role — often, the best results come from combination protocols, where secretome or exosome therapy enhances graft take and accelerates post-transplant healing.
What the Research Shows
Emerging studies demonstrate that both autologous secretomes and exosomes can:
Increase hair density and thickness
Prolong the growth phase (anagen)
Improve scalp vascularity
Reduce inflammation around follicles
The difference lies in precision — autologous secretomes deliver a patient-specific regenerative signal, while exosomes offer broader, less individualized stimulation.
